MISSION STATEMENT

Willow Lane, the Rouse Home’s Specialized Behavioral Health Unit, is dedicated to providing excellent, comprehensive, medical services for nursing home eligible residents with psychological, behavioral and psychosocial needs. This unique Household Behavioral Unit is designed for overall maximum resident safety and enhanced quality and dignity of life.

WILLOW LANE SPECIALIZED SERVICES

  • PSYCHIATRIC SERVICES: Provided by Deerfield Behavioral Health of Warren Physicians, CRNP and RNBC. These services are provided on site weekly with psychiatric coverage available 24/7 for consultation and emergency services.
  • UNIT STAFF: All staff assigned to the Unit has received specialized training in crisis prevention intervention techniques.

  • INDIVIDUAL TREATMENTS: Treatments are determined utilizing a team centered holistic approach. All treatment plans are individualized and are developed by the Unit Interdisciplinary Care Plan Team.

  • ON SITE MEDICAL SERVICES: Laboratory, x-ray, podiatry, optometry, audiology, dentistry, wound care, diabetic management and pacemaker monitoring are provided on site by credentialed agencies.

  • THERAPIES: On site professionally licensed Physical Therapist, Occupational Therapist and Speech Language Pathologist who assist residents in achieving their maximum potential.

  • REFERRAL SYSTEM: Liaison with outside referral sources to facilitate individuals receiving care in the least restricted environment.
  • RECREATIONAL SERVICES: Specialized recreational programming on site provided by trained Activity staff.

  • RESTORATIVE NURSING: Individualized restorative nursing programs provided for residents requiring these services.

  • PASTORAL COUNSELING: On site spiritual counseling available from our facility chaplain.

  • MEDICATION MANAGEMENT: On going education for residents and their families.

  • AREA AGENCY ON AGING: Community liaison and Ombudsman service.

  • TRANSPORATION: Supervised transportation to necessary out of facility appointments provided.

  • TRANSITIONING: Utilizing a multidisciplinary team approach to transition residents to other levels of care within the facility or tertiary facilities when appropriate.

WILLOW LANE AMMENITIES

Willow trees by nature are graceful displaying long, elegant, flowing limbs. Standing beneath the willow one can feel the strength and security the willow tree offers. While looking up at the trunk of the willow tree, in its immense structure, one sees how the willow branches arise like a fountain of water, drawing strength from deep within the earth, and flowing gently over the sides. A relaxed and natural peace spreads out from the limbs, with the shade providing a feeling of safety or a place of protection.

Our unique 19 bed household neighborhood environment, Willow Lane at the Rouse Warren County Home, will offer its residents a safe, positive, healing environment much like that of the willow tree where dignity and quality of life can grow.

  9. Comfortable, warm and welcoming semi-private rooms with individualized temperature controls and attached half bathroom.

  10. Full service Beauty/Barber shop.

  11. Computer service/use.
