Level of Elder Care Service Definitions

Each resident at the Suites at Rouse is assessed upon admission and periodically throughout their stay to determine the appropriate level of Elder Care Services required to meet their needs.

Level I
Level I care is considered the basic service package that is offered to all Suites residents. Each resident can expect at least one hour of Elder Care Services per day, 75% of these services being delivered during the waking hours. Level I residents must be physically and mentally capable of understanding and carrying out instructions to independentaly vacate the building in an emergency. The following Elder Care Services are provided under Level I:

  • Securing Health Care
  • Assistance making appointments
  • Supervision of personal hygiene
  • Shopping when resident can determine needs
  • Intermittent and minimal assistance with self-medication (including help with ordering meds from the resident's pharmacy or from the contract pharmacy)
  • Oversight and supervision of care
  • Social, recreational and leisure activities
  • Assistance with self-care oxygen

Level II
Level II care includesall services provided unter Level I, as needed, plus the additional care services listed below. Level II residents must be physically and mentally capable of understanding and carrying out instructions to independently vacate the building in an emergency.

  • Assistance with correspondence
  • Special skin care and monitoring
  • Medication administration (including ordering of meds with contract pharmacy or family ordering and obtaining meds)
  • Occasional assistance with incontinence for residents alert and aware of their incontinence.
    Note: incontinence products are supplied by the resident
  • Occasional assistance with ambulation, including wheelchair, to meals or activities
  • Assistance with personal hygiene/bathing
  • Non-self care oxygen

Level III
Level III care includes all services provided under Level I and II, as needed, plus the additional care services listed below. Level III residents must be physically and mentally capable of understanding and carrying out instructions to independently vacate the building in an emergency.

  • Daily care of personal possessions and clothing
  • Assistance with determination of shopping needs
  • Accompaniment to routine medical appointment
  • Routine assistance witih incontinence
  • Colostomy / Ileostomy / Ureostomy monitoring and care
  • 24-hour supervision

Level IV
Level IV care is the most enhanced care provided at the Suites prior to recommending a higher level of care ina nursing facility or other appropriate setting. Level IV care includes all of the services available under Levels I, II, III as needed, and the following:

  • Combination of above listed care that exceeds 60 minutes per day
